Our supported living service helps people to get the most from life and reach their goals.
This service is aimed at people who would find it difficult to live independently and participate in the community without support. This may be as a result of illness, disabilities, getting older, or a crisis in their life.
The wishes of each individual client are central to what we do. We respect people’s needs and preferences and we seek to offer choices about how these could be met. Our care workers work with each client to develop their individual support plan, to ensure they get the best from their care.
Transform staff work with clients to identify their goals – and then support them to achieve them. We work alongside each client to develop a support plan, which sets out the areas where the person needs help.
We provide practical support to assist clients to participate in the community, assisting with transport, personal care and communication needs. We also offer encouragement to help people to gain confidence and increase their self-esteem.
We support people to achieve their goals in a wide range of areas, including:
- Getting out and about in the community
- Taking part in hobbies and leisure activities
- Meeting new people
- Seeing friends and family
- Entering education or training
- Signposting to services in the community
- Going to appointments
- Making life choices
- Personal care
- Help with letters, emails and phone calls
- Living more independently
